What caused it? It could not have been a Myddraal blade because he would have died. Unless, of course, there happened to be an Aes Sedai in Whitebridge that was never mentioned. More so, what caused the flashes of light? I think it may have been that Thom's knives are made by the Power. So when he threw them, the Myddraal deflected them with his blade. We know from Rand's Heron Marked blade that when it hits and Myddraal's blade it creates a flash of light. For his wound, maybe a knife got through and the Myddraal threw it back at Thom and got him in the leg.
Any other ideas?

I'd say it's from the Myrdraal blade. I've never read where a scratch would kill you instantly, though I know the blades are deadly. So perhaps all he recieved was a scratch, and that's why it's never healed, i.e. why he continually limps around. As for the flash, I have zero doubt that it came from the collision between Thom's Power-wrought knives and the Myrdraal blade.

It's true that it never says that one scratch will kill you. Maybe's just a scratch.
EDIT:
Just to let you know, I made this topic at Dragonmount also. I wanted to know what they said about it. Here's the overview so far.
Thom's daggers were not Power made. And the flashes happened before the started fighting the Myddraal. They also said that the Myddraal blade's don't last forever and need replenishing. So maybe that blade's 'life span' was ending and didn't cause as much damage as a normal blade would have.

Or maybe the Fade broke his knee. I don't know, I read this in your topic on Gamefaqs as well. I think something is fishy...

I dunno, the idea of the myddraal's blade having a limit on it's use makes sense, and if that is true, then to me it makes sense that it didn't kill him instantly. Maybe it is killing him more slowly though, I mean to one degree or another it's still a fade's weapon, and thus would still kill with even a glancing blow.

It could have been what wouldn't have been a serious wound, but considering it was made by a Myrddraal blade it developed into one and was never dealt with and is past dealing with.

The blades of Thakan'dar almost always cause fatal inuries if Aes Sedai Healing is not administered and it is highly unlikely that an Aes Sedai was in white bridge at the time thom was fighting the Myrddraal so he couldn't have been stabbed by the Myrddraal because he would have died eventually even if it were just a scratch, heres what i think, thom attacks the Myrddraal with his knives while Rand and mat run away the Myrddraal deflects thoms normal knives with his sword and one of them hits thom in the leg the and he falls over (possibly breaking the same leg) the Myrddraal doesn't kill thom because its being pressured by the forsaken and runs off to find Rand and matt but thom held him off long enough for them to escape so here thom is laying in the road with a wounded (possibly broken) leg thom wraps his wound and leaves and gets an inn until his leg heals only it doesnt heal properly and leaves him with a limp then thom (now with a limp) leaves whitebridge for Cairhien.
Well what do you guys think, any thoughts?

XD indeed it was. However, for the Myrdraal being mysteriously summoned away by the Forsaken would be most convenient for Thom, and that's not Jordan-style My theory is a little far fetched, but I think Thom can channel. Why else would he be so upset about his cousin being caught by Red Ajah? He probably knows a lot about it, too, so he can hide it from Rand from time to time. So, perhaps that's what caused the flashes: Thom channeling.

It was his nephew and he was upset because his nephew could channel.
The flashes are debatable.
